Community Health Nurse (CHN)

The Community Health Nurse (CHN) is responsible for providing, teaching, supervising, and documenting nursing care for individuals and families at home. The CHN plays a vital role in promoting, maintaining, and restoring health, preventing illness, and facilitating rehabilitation.

Key Responsibilities

  • Intervene to promote, maintain, or restore health and prevent illness.
  • Collaborate with healthcare providers, professionals, and community representatives to coordinate home care.
  • Evaluate individual and family responses to interventions.
  • Function independently under the general guidance of the Director while adhering to specific guidelines and protocols.
  • Utilize time management, communication, planning, delegating, and problem-solving skills in care provision.
  • Demonstrate critical thinking and maintain emotional stability in emergency settings.

Qualifications

  • One year of acute nursing care experience required; hospital-based experience preferred.
  • Experience in home health care is required.
  • Certified Home Health license is required, including proficiency with OASIS.
  • Infusion Therapy Certification preferred.
  • Demonstrated knowledge of nursing principles and practices.
  • Ability to apply community health principles and practices effectively.
  • Proven ability to perform independently as a clinician with minimal supervision.
  • Experience with self-scheduling of caseload preferred.
  • History of flexibility in assuming varied assignments.
  • Bachelor of Science in Nursing (BSN) is highly preferred.
  • Current Driver License and insurance are required.
  • Familiarity with home care reimbursement and regulatory guidelines is essential.
  • Ability to drive in inclement weather.
  • Must be willing to work weekends and holidays as assigned.

Additional Information

All requests for time off must be noted and verified prior to submission. A maximum of five days of requested time off is acceptable. Non-billable orientation of 8 hours is required for this position.
